Service-Learning Opportunity Questionnaire

Volunteer Insights works with faculty and community agencies to connect students with service opportunities that match learning objectives.

If you are in Central Oregon and would like us to recruit students to serve with your agency, please complete this questionnaire for each opportunity that fits the listed service-learning criteria. To respond by mail, please “Print this Form” and send the completed form to Volunteer Insights, PO Box 5125, Bend, OR 97708. To respond online, please fill in the form below and click “Submit.”

Criteria for Service-Learning Opportunities

  • Central Oregon non-profit or public agency serving community or environmental needs (or for-profit agency, such as a hospital, whose purpose is serving a social need).
  • Agency provides service opportunities that can fit students' learning goals.
  • Agency has volunteer assignments (one-time or weekly) that can be completed within one quarter (up to ten weeks), with the possibility of continuation beyond the term.
  • Agency incorporates risk-management procedures for injury prevention and liability reduction.
  • Agency screens volunteers, as needed.
  • Contact person, volunteer manager, and/or supervisor will:
    • Respond quickly to student inquiries and let them know quickly whether they are needed
    • Place students in service opportunities that connect their educational goals with agency/client needs
    • Ensure that students are oriented, trained, and directly supervised
    • Record student hours
    • Communicate with students and/or their instructors about problems and successes
    • Assist students to connect their service experience with their learning goals
