The following service-learning forms are available for use by service-learning faculty, students, and community agency partners, to facilitate preparation and evaluation.
Service-Learning Agreement
Service Learning Agreement (PDF)
Purpose:
- To clarify expectations for all participants
- To define and share learning objectives
- To keep a record of students involved in service-learning
- To facilitate communications with community partners
- To promote safety and reduce risk
Procedure:
- Complete the Instructor section at the bottom of the Agreement.
- Then, copy BOTH SIDES of the agreement to give to students.
- Work with your students to fill in the learning goals for the service experience. Site coordinators appreciate this information, as it helps them place students.
- Have students read both sides of the Service-Learning Agreement in class, and go over the requirements with them. This is crucial for risk management.
- Ask the students to take the Agreement to the site and return the signed form to you, preferably by two weeks after the start of the quarter. (Note: Some sites will not accept service-learning students after the second week of the quarter.)
- Ask agency coordinators to work with the students to fill in the service goals. If possible, agencies should make copies to keep on site.
- Keep completed Agreements for at least two terms beyond the course.
Evaluation by Agencies
Evaluation by Agencies (PDF)
Purpose:
- To account for students' successful completion of service assignments
- To offer agency input in the service-learning partnership
- To provide faculty and students with feedback on their service
Procedure:
- Complete the Instructor section, including the due date (so that you will receive the form back before the end of the quarter).
- Copy the form (or a modified one of your own) for students to take to their supervisors.
- Be sure that students ask their supervisors to complete the evaluation form before the end of the term.
Evaluation by Students
Evaluation by Students (PDF)
Purpose:
- To assess students' service-learning experiences
- To improve Volunteer Insights' list of volunteer opportunities
- To evaluate and improve course reflection activities that connect service with learning
Procedure:
- Students complete the evaluation forms in class at the end of the quarter.
